Kinship Care Resources & Affidavit: Below is the Kinship Care Fact Sheet for the State of Maryland as well as the Consent for Healthcare Affidavit. The Consent for Healthcare Affidavit is available to relative caregivers who do not have legal custody or guardianship of the relative child in their care. The caregiver must be able to verify the relationship with the child and that the child’s parent(s) is not available.
