The Baltimore City Health Department's Population Health and Disease Prevention division is committed to enhancing the City of Baltimore's health through proactive and wrap-around services. We manage a wide-range of programs to address health impacts and threats to our community. Programs within Population Health and Disease Prevention include Communicable Diseases; Avian Influenza, Measles, Hepatitis A, Hepatitis C, and Flu to reduce incidence's in Baltimore City through prevention, surveillance, investigation, and control of disease and outbreak occurrences. The Overdose Prevention program aim to reduce substance use-related harm, while clinical services and HIV Prevention focus on providing critical health care and support to residents within our community. 

The BCHD Community Risk Reduction strategies work to minimize health hazards, and Field Health Services deliver essential care directly to those in need. Through Population Health and Disease Prevention, BCHD continue to work to safeguard and improve public health through dedicated and targeted efforts.
